Please fill out the following form in order to participate in our activities. This agreement releases Rejoice Fitness from all liability from all injuries that may occur during a session. By signing this agreement, I agree to hold Rejoice Fitness free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are a result of negligence. I understand that I am committing to a physical activity program that may include the following:
• STRENUOUS ACTIVITY
• MUSCLE AND STRENGTH TRAINING
• ENDURANCE TRAINING
• CARDIOVASCULAR CONDITIONING
• OTHER VARIOUS FORMS OF FITNESS
I understand there are risks associated with partaking in a physical activity program. I will communicate any recent injuries or modifications I need. I am acknowledging that I do not have any conditions that will hinder my participation.
